If you need to submit a sick note request, request test results, GP letter or if you have a general question or concern regarding your health we have created an easy questionnaire for you to complete and this will get sent through to the practice.

We ask that you allow at least 3 working days for all admin requests to be dealt with. If you have an urgent query or concern please do not use this service and we ask you contact our reception team by telephone 

We do not accept emails from patients and our email is strictly for clinical staff only.
